Requirements
- Active Missouri RN or LPN license (or Compact License)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Medication administration and management
- Monitor and document patient condition
- Perform treatments as ordered by the physician
- Educate and support family members
- Work closely with the Phoenix clinical team for ongoing support
