About The Position

Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) in Apex, NC and surrounding areas. The role involves providing direct, one-on-one medical care to medically fragile infants, children, and/or adolescents primarily in their home. Angels of Care, established in 2000, operates on values of Heart, Advocacy, Love, Outreach, and Speed (HALOS), aiming to make a significant difference in the lives of patients and their families through dedicated medical care.

Requirements

• Active RN or LPN/LVN license
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care
• Educate family members on patient clinical care to enhance positive outcomes
• Preventative initiatives to protect quality of care for patient
