About The Position
Tired of running from room to room? Provide true one-on-one care in a patient’s home — the way nursing was meant to be: relationship-based, focused, and meaningful! One patient at a time — no facility chaos or overloaded assignments. Consistent schedules and long-term cases. Work close to home while building real bonds with patients and families. Become part of someone’s support system during the moments that matter most.
Requirements
- Active Missouri RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
