Requirements

• Active IL RN or LPN license (or Compact license)
• Strong assessment and communication skills
• Compassionate, dependable, and self-motivated
• Valid driver’s license and reliable transportation
• A great fit for nurses burned out from facility work, those seeking meaningful patient relationships, semi-retired nurses, and clinicians who want to serve their local community

Responsibilities

• Provide skilled, one-on-one nursing care in the home
• Administer and manage medications
• Monitor and document patient conditions
• Perform treatments as ordered by the physician
• Educate and support family members
• Collaborate closely with the Phoenix clinical team
