About The Position

Requirements

  • High School (Required)
  • BLS or CPR certification within 90 days of hire. - Sarah Bush Lincoln
  • Graduate of Accredited LPN Program - Sarah Bush Lincoln
  • LPN - Licensed Practical Nurse - Illinois Department of Financial and Professional Regulation

Responsibilities

  • Administers medications and treatments according to protocol.
  • A plan of care is initiated at admission with focus on patient’s identified physiological, psychological, and environmental factors.
  • Identifies patient problems on integrated flowsheets and documents against the plan of care.
  • Coordinates discharge planning with all team members.
  • Demonstrates the ability to handle emergency situations in a prompt, precise and professional manner.
  • Documents accurate and ongoing assessments of patient status every 2 hours or as needed.
  • Documents patient teaching/education about plan of care.
  • Initiate appropriate pathways of inpatient physician orders.
  • Monitors that each caregiver is productive and serves as a source to provide and/or recommend education if needed.
  • Notifies team members that an assessment is required as per physician order.
  • Performs patient assessments each shift of work and documents at time of assessment.
  • Provides and coordinates patient and family education and documents appropriately.
