Requirements
- High School (Required)
- BLS or CPR certification within 90 days of hire. - Sarah Bush Lincoln
- Graduate of Accredited LPN Program - Sarah Bush Lincoln
- LPN - Licensed Practical Nurse - Illinois Department of Financial and Professional Regulation
Responsibilities
- Administers medications and treatments according to protocol.
- A plan of care is initiated at admission with focus on patient’s identified physiological, psychological, and environmental factors.
- Identifies patient problems on integrated flowsheets and documents against the plan of care.
- Coordinates discharge planning with all team members.
- Demonstrates the ability to handle emergency situations in a prompt, precise and professional manner.
- Documents accurate and ongoing assessments of patient status every 2 hours or as needed.
- Documents patient teaching/education about plan of care.
- Initiate appropriate pathways of inpatient physician orders.
- Monitors that each caregiver is productive and serves as a source to provide and/or recommend education if needed.
- Notifies team members that an assessment is required as per physician order.
- Performs patient assessments each shift of work and documents at time of assessment.
- Provides and coordinates patient and family education and documents appropriately.
