About The Position

Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) in Phoenix, AZ and surrounding areas. Nurses in this role provide direct, one-on-one medical care, making a significant difference in the lives of their patients and families. The position involves focusing on the medical needs and treatment of infants, children, and/or adolescents primarily in their home, adhering to the active plan of care provided by physicians and other personnel. Key responsibilities include conducting ongoing patient care and assessments, administering prescribed medication, treatments, and therapies, coordinating care, educating family members to enhance positive outcomes, and implementing preventative initiatives to protect the quality of patient care. Requirements

• Active RN or LPN/LVN license (New Grads Welcome, training provided!)
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care.
• Educate family members on patient clinical care to enhance positive outcomes.
• Preventative initiatives to protect quality of care for patient.
