About The Position

Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and/or adolescents primarily in their home setting in Glendale, AZ and surrounding areas. Nurses are responsible for focusing on patient medical needs and treatment according to the active plan of care, conducting ongoing patient care and assessments, administering prescribed medication, treatments, and therapies, coordinating care, educating family members on clinical care, and implementing preventative initiatives to protect the quality of care. The role offers full-time and part-time shifts, including days, nights, and weekends.

Requirements

Active RN or LPN/LVN license (New Grads Welcome, training provided!).
Provide care in a client home setting.

Responsibilities

Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
Conduct on-going patient care and assessments.
Administration of prescribed medication, treatments, and therapies.
Coordination of care.
Educate family members on patient clinical care to enhance positive outcomes.
Preventative initiatives to protect quality of care for patient.
