About The Position

The position involves monitoring the Electronic Health Record nurse pool throughout the day and responding in a timely manner. The role requires triaging incoming calls, messages, and In-Basket tasks, prioritizing urgent matters, and scheduling appointments as needed. The nurse will exercise nursing judgment and standard triage guidelines to ensure the health needs and concerns of patients are met. Responsibilities include assessing complex patient needs, ensuring appropriate transitions to levels of care, and providing clinical support to the team. The nurse will also document all calls and encounters in the patient's medical record and educate patients about community services. Collaboration with the interdisciplinary care team is essential to avoid interruptions in service delivery.

Requirements

  • Ability to read, write and communicate in English
  • Graduate of an accredited Practical Nursing program or an accredited school of nursing
  • Currently licensed as a Licensed Practical Nurse or Registered Nurse in the state of Massachusetts
  • Basic Life Support Certification for Health Care Providers
  • Strong interpersonal, customer service, and time management skills
  • Experience with current office technology and proficient with computer office applications
  • Minimum of two years’ experience in a clinical setting preferred

Responsibilities

  • Monitor the Electronic Health Record nurse pool throughout the day and respond in a timely manner
  • Triage incoming calls, messages, and In-Basket tasks, prioritizing urgent matters and scheduling appointments as needed
  • Exercise nursing judgment and standard triage guidelines to ensure health needs and concerns of patients are met
  • Assess complex patient needs to ensure appropriate level of care transitions
  • Collect objective and subjective data and assess patient responses
  • Triages and schedules visits based on patient disposition needs and available resources
  • Provide clinical support to the team, adjusting role and priorities as needed
  • Answer phone calls and electronic messages promptly
  • Greet callers appropriately and provide assistance in a friendly manner
  • Provide timely and accurate triage responses according to professional nursing standards
  • Document all calls and actions taken into the patient’s medical record timely and completely
  • Educate patients about utilization of community services
  • Collaborate with the interdisciplinary care team to ensure continuity of care
  • Perform other work-related duties as assigned
  • Greet and escort patients to exam rooms
  • Obtain relevant information and take vital signs as indicated
  • Input clinical information into the electronic health record timely and accurately
  • Assist clinicians with clinical procedures and tests specific to the department
  • Advise patients of preparation required for exams or additional testing
  • Assist in resolving patient concerns by acting as an advocate
  • Participate in practice and department meetings and trainings
  • Take initiative to learn and stay current with community resources
  • Maintain patient rights and confidentiality obligations
  • Provide clerical/office support as necessary
  • Float to other MACC practices when required
  • Verify and implement clinician’s orders for testing and procedures
  • Triages walk-in patients per established protocols
  • Maintain communication with the interprofessional team to facilitate safe transitions of care
