About The Position

The Care Team Navigator will ensure that Ohio’s Hospice patients and families receive excellent care and services. The Care Transition Navigator will coordinate care and provide support to patients, families, and Ohio’s Hospice teammates. As a member of our team, you'll have a chance to impact many lives. Requirements

  • RN’s - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Ability to provide direct patient care
  • Two years acute care nursing experience preferred
  • Must be able to pass a criminal background check
  • Must be able to pass a 10 panel drug screen

Nice To Haves

  • Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio’s Hospice employees who wish to attain certification.
