About The Position

JOB SUMMARY: The LPN will play a vital role in providing high-quality care to individuals with autoimmune and musculoskeletal conditions. This position involves close collaboration with rheumatologists and other healthcare professionals to support patients through diagnosis, treatment, education, and on-going management of chronic illnesses.

Requirements

  • Current LPN licensed in Georgia required.
  • Minimum of 1-2 years clinical experience (experience in rheumatology, immunology, or chronic disease management preferred).
  • Strong communication, organizational, and critical thinking skills
  • BLS certification required
  • Ability to communicate in English, both verbally and in writing.
  • Strong written and verbal skills.
  • Basic Computer Skills
  • Follows Code of Conduct policy.
  • Adheres to dress code; appearance is neat and clean.
  • Completes annual educational requirements.
  • Maintains regulatory requirements.
  • Maintains patient confidentiality at all times.
  • Reports to work on time and as scheduled; completes work within designated time.
  • Wears identification when on duty; uses computerized time clock system correctly.
  • Completes in-services and returns in a timely fashion.
  • Attends annual review and/or skills fair and department in-services, as scheduled.
  • Complies with all organizational policies regarding ethical business practices.

Responsibilities

  • Provide direct patient care including assessments, medication administration, and treatment monitoring for rheumatology patients.
  • Assist with in-office procedures and infusion therapies as needed
  • Educate patients and families on chronic disease management, medications, and lifestyle modifications
  • Coordinate care with physicians, advanced practice providers, and other healthcare team members
  • Monitor patient progress and respond to changes in condition or treatment needs
  • Maintain accurate and up-to-date clinical documentation in electronic health records
  • Triage patient phone calls and provide appropriate follow-up and support
