About The Position
Under the direction of the RN, ensures high quality medical care and services by coordinating, scheduling, communicating and documenting medical appointments and medical issues for consumers. Includes transportation to and from appointments.
Requirements
- LPN
- Licensed by the NYS Department of Education as LPN.
- NYS drivers' license.
- Must have access to timely and reliable transportation to transport self to multiple sites.
Nice To Haves
- Experience in the field of intellectual and developmental disabilities preferred.
