About The Position

Licensed Practical Nurse (LPN) / Resident Care Coordinator (RCC) Schedule: Sunday–Thursday Pay Range: $33–$36 per hour (based on experience) Location: Assisted Living / Memory Care Community We are seeking an experienced and dependable LPN / Resident Care Coordinator (RCC) to join our clinical leadership team. This role is hands-on and requires someone who is comfortable balancing resident care coordination, medication oversight, and floor support when needed. This is a working nurse role, not a desk-only position.

Requirements

  • Current Colorado LPN license in good standing (required)
  • Strong medication management and clinical judgment skills
  • Ability and willingness to assist with hands-on resident care
  • Strong documentation and communication skills
  • Ability to remain calm, organized, and decisive in a fast-paced environment

Nice To Haves

  • Experience in Assisted Living, Memory Care, or Long-Term Care preferred
  • Experience supervising or leading care staff strongly preferred

Responsibilities

  • Provide direct nursing care to residents in Assisted Living and/or Memory Care
  • Administer medications and treatments in accordance with physician orders and state regulations
  • Serve as RCC by coordinating resident care plans, assessments, and clinical documentation
  • Supervise and support Caregivers/QMAPs during assigned shifts
  • Assist on the floor as needed with resident care and staffing support
  • Communicate effectively with families, providers, and internal leadership
  • Ensure compliance with Colorado Department of Public Health and Environment (CDPHE) regulations
  • Participate in audits, surveys, and quality assurance processes
  • Respond to clinical concerns, incidents, and emergencies appropriately
