Behavioral Health Connections (BHC) in Dunwoody, GA is seeking a dynamic and talented LPN Referral Management Weekday Full Time Overnights 10 hours Shifts that will review assessment referrals and work closely with the RN and other clinicians, to determine the appropriate placement of patients, seeking treatment. Assist the call center specialist with other duties, to enhance referral management process.

Requirements

• Nursing Degree, preferred Bachelors or Masters.
• Licensed LPN in current State or compact license.
• Current CPR, if required by a customer hospital.
• Preferred 1-3 years related work experience in Behavioral Health.

Responsibilities

• Review assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care.
• Maintain patient records, updating databases, and ensuring accurate documentation of patient information.
