The SPIRE study is seeking a full time LPN Program Coordinator for patients who are enrolled in the GRACE arm of study. 

Requirements

  • Current, unrestricted Licensed Practical Nurse (LPN) license (state-specific)
  • Graduation from an accredited practical nursing program
  • Knowledge of outpatient clinical workflows
  • Basic computer skills and experience with EHR systems
  • Strong communication, organizational, and documentation skills

Nice To Haves

  • Experience conducting Medicare Annual Wellness Visits
  • Familiarity with CMS preventive care guidelines
  • Outpatient primary care or internal medicine experience
  • Bilingual skills preferred

Responsibilities

  • Check patients in/out in the EMR System
  • Schedule appointments
  • Coordinate patient care with outside agencies such as mobile imaging, blood draw, VNAs, etc.
  • Maintain GRACE patient by conducting items such as monthly Chronic Care Management, TCM calls, scheduling TCM appointments and ER Discharge appointments.
  • Monitor GRACE patient implemented care plans and coordinate care such as referrals, hospitalizations, patient education, etc.
  • Call patients with results
  • Manage DME orders, prior authorizations, home care orders, and clinical paperwork (FMLA, Medical Transportation, Assisted Living, etc.)
  • Triage patient medical complaints/concerns.
  • Conduct Medicare Annual Wellness Visits in accordance with CMS guidelines under provider supervision
  • Collect and document comprehensive patient histories, including:
  • Medical, surgical, family, and social history
  • Medication reconciliation (prescription, OTC, supplements)
  • Allergies and immunization status
  • Perform and document required screenings:
  • Depression screening (PHQ-2/PHQ-9)
  • Cognitive impairment screening
  • Fall risk assessment
  • Health risk assessment (HRA)
  • Tobacco, alcohol, and substance use screening
  • Record vital signs, height, weight, BMI, and other relevant clinical data
  • Identify preventive care gaps and assist with development of personalized prevention plans
  • Provide patient education on preventive services, chronic disease management, and lifestyle modifications
  • Ensure accurate and timely documentation in the electronic health record (EHR)
  • Assist providers with outpatient clinical workflows as needed
  • Prepare patients for provider visits and procedures
  • Administer injections, immunizations, and medications per scope of practice and provider orders
  • Perform basic clinical procedures consistent with LPN licensure
  • Coordinate referrals, diagnostic testing, and follow-up appointments
  • Communicate test results and care instructions to patients per protocol
  • Maintain exam room cleanliness and stock supplies
  • Adhere to infection control, safety, and HIPAA regulations
  • Support quality initiatives, population health programs, and preventive care metrics
  • Collaborate with providers, care coordinators, and administrative staff
  • Demonstrate compassionate, patient-centered care and professionalism
  • Additional responsibilities required
  • Floating to other GFP offices may be required
