The Pre-Admission Coordinator is a key role responsible for reviewing patient records before admission to ensure all necessary clinical documentation and critical information for billing and care delivery are present. This role involves managing pending admissions, whether patients are awaiting discharge or surgery, and coordinating with sales and referring facilities to ensure patients are informed and convert to Elara Caring's services. Requirements

  • Degree from an accredited school of professional nursing
  • Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
  • 1 year of practical experience
  • Ability to convey information and opinions, listening to others and checking for understanding, responding in appropriate message and tone
  • Ability to set timeframes, match resources to tasks, and plan ahead
  • Ability to make decisions with incomplete information, choosing from multiple options, knowing when to escalate
  • Ability to quickly become proficient in enterprise applications (e.g., Homecare Homebase, Workday), intermediate knowledge of Microsoft Office
  • Ability to handle complaints, manage conflict and negotiate outcomes
  • Ability to seek patient feedback, advocate for patient, understanding of patient needs, and managing patient expectations
  • Valid driver’s license and insurance and reliable transportation to perform job tasks

Nice To Haves

  • Home Health or Hospice experience is preferred
  • Bilingual in English and Spanish is preferred

Responsibilities

  • Review patient records prior to admission to ensure adequate clinical documentation and other critical information required for billing and care delivery have been provided
  • Manage an area’s pending admissions, whether they are awaiting discharge or pending surgery
  • Coordinate between sales and the referring facility to ensure patient is informed and converts
