The LPN Per Diem will participate in data collection related to patient status and needs, inform and instruct patient/caregivers on patient's health/illness and Plan of Care, ensure all procedures are performed based on established Standards of Care, document all aspects of patient care, teach patient/significant other about health/illness and plan of care, and be a team player and take on other responsibilities as needed.
