About The Position

The Private Duty Pediatric LPN at Southwestern Home Health is responsible for providing skilled in-home care to pediatric patients. This role involves one-on-one patient interactions, assisting with specialized procedures, and ensuring the delivery of high-quality care in accordance with agency policies. The position offers flexible scheduling options and the opportunity to work closely with patients and their families.

Requirements

  • Current LPN or RN license through the board of nursing in the state of practice.
  • Positive attitude and good organizational skills.
  • Self-directed with the ability to work with little supervision.
  • Flexible and cooperative in fulfilling role obligation.
  • Complies with accepted professional standards and practice.
  • Possesses and maintains good physical stamina and mental health.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.

Nice To Haves

  • At least one year pediatric nursing experience.
  • Community/home health or medical/surgical experience.

Responsibilities

  • Provide safe home health services to patients/clients and families/caregivers.
  • Implement plan of care initiated by the RN Clinical Supervisor in collaboration with the physician.
  • Assist the registered nurse or physician in carrying out the plan of care.
  • Develop, prepare, and maintain individualized patient/client care documentation consistent with the plan of care, and submit accurate documentation.
  • Assess and provide patient/client and family/caregiver education and information pertinent to diagnosis and plan of care as established by the registered nurse.
  • Document communication and condition of the patient and address problems identified in the ongoing assessment or provide documentation of rationale for not doing so.
  • Assist the patient/client and family/caregiver in learning home nursing procedures, self-care techniques, and other care needs as appropriate to the patient's/client's condition.
  • Administer medication, diet, and other instructions as directed by the registered nurse and ordered by the physician.
  • Use equipment and supplies effectively and efficiently.
  • Participate in personal professional growth and development and keep current in nursing trends and knowledge.
  • Participate in in-service programs and quality improvement program of the agency.
  • Participate in public relations and community activities that promote the agency's role as an effective member of the health care delivery system.
