About The Position

The position requires an LPN License and is primarily located in Calhoun, with the necessity to travel to Chatsworth. The role involves scheduling home visits for field staff and assisting in the coordination of patient care. The individual will work under the direction of the Clinical Manager to ensure the proper utilization of clinical resources, maximizing care center capacity and adhering to utilization guidelines. Responsibilities include scheduling and communicating with field staff, patients, and the Clinical Manager regarding patient services.

Requirements

  • Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to the state of assignment.
  • High School diploma or GED equivalent.
  • One (1+) year of administrative experience in a health care environment.

Nice To Haves

  • One (1+) year of scheduling experience in a healthcare environment.

Responsibilities

  • Ensures patient assessment visits including all Oasis visits are scheduled and performed timely.
  • Prepares weekly clinician schedules and reviews them daily to determine capacity opportunities and ensure productivity standards are met.
  • Assigns and schedules clinicians in the most efficient geography, maximizing clinician efficiency and matching skill with required care.
  • Ensures missed visits are scheduled by the end of the current week or referred to the Clinical Manager for resolution.
  • Works collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered.
  • Manages calls from patients and field staff related to scheduling issues, referring clinical and performance issues to the Clinical Manager.
  • Monitors hospitalized patients and ensures the team follows up as necessary.
  • Completes requested schedules ensuring all orders and services are included as required.
  • Supports the Clinical Manager and ensures timely communication with patients, clinicians, referral sources, and other personnel.
  • Runs applicable reports and responds to workflow taking appropriate actions.
  • Assists with internal or external transfer of patients between care centers and/or Hospice services.
  • May provide direct patient care in emergencies if a licensed clinician.
  • Maintains supply closet with routine supplies and serves as liaison with the field for patient supply needs.
  • May be responsible for the referral and intake process in the absence of an Intake Coordinator.
  • May obtain verbal orders for patients and document as applicable.
  • Performs other duties as assigned.
