Requirements

  • Current licensure as a License Practical Nurse with the Ohio Board of Nursing.
  • At least one year of experience working with individuals with physical or behavioral health disabilities in a home health care, medical, or behavioral health environment is required.
  • Must have a current valid Ohio driver’s license, proof of auto liability insurance.
  • Must be able to travel in own personal vehicle within the 5-county WRAAA region during work hours.
  • Ability to accurately and timely document in members’ electronic record
  • Knowledge of use of general office equipment and computer skills.
  • Proficiency in interviewing skills, listening and excellent interpersonal skills.
  • Excellent written communication skills with the ability to complete documentation in grammatically correct format.
  • Strong organization skills with the ability to prioritize.
  • Ability to work independently within the team structure.
  • Flexibility to adjust to variations in the work environment.
  • Ability to work with multi-cultural members and experience with an interpreter.
  • Knowledge of medical terminology and basic disease processes.
  • Comfortable in going into members’ homes, hospitals and nursing facilities.
  • Must be detail oriented, have excellent interpersonal communication skills and the ability to work effectively with others.

Responsibilities

  • Acts as a liaison between Care Management staff, member and providers.
  • Monitors the progress of home modifications through completion, communication with providers and members, and confirming satisfaction with modification via a home and/or telephonic contact/visit.
  • Provides in-office telephone coverage and triaging with Care Manager Supervisor regarding the disposition of calls.
  • Assists with completing Emergency Response Outreach to members at the request of the MCO during weather events or other emergency situations. (MCO= Managed Care Organization)
  • Completes claims review of program member’s services, including follow up with members and providers when discovery is made of any discrepancy between services authorized and delivered.
  • Assists in obtaining discharge summaries following members’ transitions of care.
  • Completes reconciliation of members Service Plans, including entering of Care Manager authorized services.
  • Assists members in closing HEDIS gaps of care though discussion on telephone calls or in-person visits.
  • Management of spreadsheets, data, and reports required by the MCO or internal management staff.
  • Assists in the review of electronic files per the direction of supervisor.
  • All staff have the responsibility and authority to propose new ways to improve quality and to fully participate in quality improvement efforts.
  • Performs other duties as assigned.
