About The Position

The LPN Patient Care Coordinator plays a crucial role in the healthcare team by providing care coordination and self-management support for patients within the physician office setting. This position is responsible for ensuring high-quality standards of care, coordinating disease registry activities, and acting as a liaison between patients, families, and healthcare providers. The coordinator will work closely with various teams to enhance patient outcomes and maintain safety and best practices across the healthcare continuum.

Requirements

  • High School Diploma plus 1-year vocational school.
  • One year of related experience in a healthcare setting.
  • Georgia Licensed Practical Nurse or Multistate Nursing license.
  • Proficient in computer programs such as Word, Meditech, and Epic.
  • Excellent verbal and written communication skills.

  • Knowledge of chronic disease management protocols.
  • Familiarity with patient advocacy and community resources.

Responsibilities

  • Coordinate functions and educational materials for back office staff related to pre-visit chart reviews and data collection.
  • Assist as a liaison with patients and their families to physicians, clinical staff, and other departments.
  • Follow up with patients by phone shortly after hospital discharge.
  • Act as a liaison with specialty clinics and community resources.
  • Assess clinic needs and collaborate with the Clinic Manager on strategies to achieve quality and efficiency goals.
  • Participate in committees such as Performance Improvement Teams.
  • Oversee and analyze data for the disease registry database, ensuring it is up to date and identifying patients overdue for visits or referrals.
  • Work with patients on self-management support, setting goals for chronic disease management, and addressing medication adherence.
  • Document all patient interactions accurately and maintain confidentiality according to HIPAA guidelines.
  • Participate in orientation and staff development activities as requested.
