LPN (Licensed Practical Nurse) Outpatient Wound Care – St Francis Medical Center Job Summary: The Licensed Practical Nurse (LPN) delivers safe and efficient care to patients using nursing interventions. The LPN is responsible for general clinical and clerical duties associated with direct and indirect patient care.

Requirements

  • Completion of an accredited LPN (Licensed Practical Nursing) Program
  • Current state licensure as a Licensed Practical Nurse (LPN) (required)
  • Basic Life Support (BLS) - American Heart Association (required)
  • Recent experience in acute care and/or home health care (required)
  • Wound Care Centers that offer HBOT will need successful completion of Undersea & Hyperbaric Medical Society (UHMS) approved 40-hour course within 6 months of hire and completion of written exam (required)

Nice To Haves

  • Wound Care Certification (preferred)
  • Hyperbaric Oxygen Certification (preferred)
  • Experience in Wound Care and Hyperbaric Oxygen Therapy (preferred)
  • EPIC Electronic Health Record (EHR) training (preferred)

Responsibilities

  • Collects and analyzes the assessment data, evaluates patient's condition, and collaborates with the provider to develop and maintain the patient's plan of care.
  • Provides education to patients, families, and staff members, as directed by the provider or manager.
  • Coordinates the care of the patient with other healthcare entities according to the patient's plan of care.
  • Administers medications consistent with the organizations policies and procedures, as well as the state requirements with which they are licensed in.
  • Other duties as assigned.
