About The Position

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. The role involves demonstrating knowledge of nursing principles for the resident in the relevant clinical setting and practicing within the full scope of nursing expertise/knowledge. The LPN utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. The position requires the ability to perform testing to evaluate data reflective of the resident's status and to provide the necessary care. The LPN ensures the health, comfort, and safety of residents, contributes to the assessment of residents, administers medications or treatments as ordered, and provides technical support to healthcare professionals as needed. Documentation of resident interactions and outcomes related to care is also a key responsibility. The LPN must establish and maintain effective working relationships with residents, healthcare providers, and the public, and communicate clearly while maintaining confidentiality of sensitive information. Counseling residents and family members on prevention and treatment plans under the direction of a registered nurse or physician is also part of the role.

Requirements

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Associate degree in practical nursing is preferred.
  • Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span.
  • Minimum of 400 hours of experience as LPN at another facility may be required.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board.
  • Obtains and maintains required department specific competencies and certifications.
  • Completes required annual continuing education units (CEU) and competency training requirements.
