About The Position

Hackley Community Care (HCC) is a Federally Qualified Health Center dedicated to providing comprehensive, high-quality care to our community. We serve youth and adults with chronic medical and mental health disorders, including serious behavioral health challenges, developmental disabilities, chronic disease, and substance use disorders. Our Integrated Health Clinic, located within HealthWest, allows us to deliver primary care and dental services to patients already receiving mental health support. This collaborative approach ensures we meet the unique healthcare needs of our patients in a holistic, patient-centered way. HCC is seeking a compassionate and skilled LPN or Medical Assistant to provide clinical support in our Integrated Health Clinic. The ideal candidate is comfortable working with diverse patient populations and can support a multidisciplinary healthcare team in delivering high-quality care. Under the direct supervision of the Site Supervisor, and in collaboration with HealthWest – Integrated Health Clinic Leadership, the Medical Assistant functions as a member of a multidisciplinary team to support Medical and Nursing Providers.

Requirements

  • Graduate of an accredited nursing or medical assistant program
  • Current, active, in-good-standing LPN license from the State of Michigan OR Current, active, in good standing Medical Assistant certification; preferred
  • CPR/Basic Life Support (BLS) certification
  • Strong written and oral communication skills
  • Ability to work independently and collaboratively
  • Proficient in computer and EHR software
  • Experience and comfort working with a diverse patient population

Responsibilities

  • Assist healthcare providers with patient care and preparation
  • Gather and document patient information, including medical history and vital signs
  • Assist with medication refills and documentation
  • Support pre-visit planning and care coordination
  • Help identify and address social determinants of health as appropriate
  • Ensure accurate and timely documentation in EHR and other clinical systems
  • Collaborate effectively with the multidisciplinary team to provide high-quality, patient-centered care
