About The Position

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in assisted living, under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles for the resident in the relevant clinical setting and practices within the full scope of nursing expertise/knowledge. They utilize appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. The LPN must possess the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. They ensure the health, comfort and safety of residents, contribute to the assessment of residents, and administer medications or treatments as ordered. The LPN also provides technical support to healthcare professionals as needed and documents resident interactions and outcomes related to resident care.

Requirements

  • Must have graduated from an accredited school or college of nursing program leading to licensure as a Licensed Practical Nurse.
  • Requires current licensure as a practical nurse in the state/states of practice.
  • Long term care, sub-acute care, medical/surgical experience preferred.

Responsibilities

  • Demonstrate knowledge of nursing principles for the resident in the relevant clinical setting.
  • Practice within the full scope of nursing expertise/knowledge.
  • Utilize appropriate age and population specific standards related to the physical and psychosocial needs of the resident.
  • Perform testing to evaluate data reflective of the resident's status.
  • Ensure the health, comfort and safety of residents.
  • Contribute to the assessment of residents and administer medications or treatments as ordered.
  • Provide technical support to healthcare professionals as needed.
  • Document resident interactions and outcomes related to resident care.
  • Establish and maintain effective working relationships with residents, healthcare providers, and the public.
  • Communicate clearly and maintain confidentiality of sensitive information.
  • Counsel residents, family members, and/or resident advocates on prevention and treatment plans.
  • Develop a self-care program based on resident education principles.
  • Perform other related duties as assigned.
