Requirements

• Active Nursing License in good standing.
• Attention to detail.
• Time management skills.
• Effective problem-solving and conflict resolution abilities.
• Good organization and effective communication skills.
• Six months of recent experience as a Nurse in a clinical care setting (preferred).
• Home health experience (preferred).

Nice To Haves

• Experience in home health care.

Responsibilities

• Deliver high-quality care to medically fragile patients in their homes.
• Utilize a personal tablet for electronic charting.
• Participate in paid training and continuing education opportunities.
• Manage a 1:1 caseload with no minimal weekly visits.
• Provide effective communication and support to patients and their families.
