About The Position

Licensed Practical Nurse The Licensed Practical Nurse (LPN). delivers exceptional service by meeting client’s needs and by ensuring all health needs, the safety of all clients and personal needs are met. The L.P.N. assists the R.N. with planning, coordinating, and managing the care service and complies with the care regulations. 32 Hours per Week Essential Functions Primary Responsibility: Manage medications, including ordering, renewals, and weekly pillbox preparation. Ensure clients meet residency requirements and recommend alternative placement if necessary. Monitor and maintain client charts, nursing notes, and documentation. Communicate changes in client conditions or needs to physicians and staff. Assist in maintaining compliance with federal, state, and local regulations, including availability for license surveys. Respond to client concerns or complaints and maintain communication with families. Maintain confidentiality of client, personnel, and vendor information. Participate in staff meetings, committees, and on-call management rotation as needed. Serve as Charge Nurse and perform rounds to monitor staff performance, unit flow, and resident status.

Requirements

  • Licensed with the State of Minnesota as a L.P.N.
  • Must have a valid driver’s license.
  • Driving may be necessary.
  • Ability to lift/carry objects weighing up to approximately 30 lbs. or more and to meet all physical requirements related to this position.

Nice To Haves

  • Experience in working with older adults.
  • Minimum of three years of experience in a related field.
