About The Position
Responsible for the coordination of the MDS process, based on the comprehensive assessments of the resident's needs.
Requirements
- Must have a minimum of two years Long Term Care experience.
- Must be a Licensed Practical Nurse in the State of Ohio.
Responsibilities
- Completes initial assessment of residents and all subsequent assessments.
- Updates physician's orders as needed.
- Responsible for entire MDS process.
- Reviews MDS assessments when there is a change in a resident's condition and/or quarterly.
- Develops Comprehensive Care Plan with the Interdisciplinary Team.
- Makes recommendations for charting requirements.
- Informs staff of any changes regarding resident care and care issues.
- Monitors "RUG" scores for optimal reimbursement.
- May be required to take on-call needs and assist with routine LPN duties.
- Maintain the confidentiality of all resident, staff and organization information.
