About The Position

We are seeking a dependable and experienced LPN/LVN for an assignment in a nursing home setting. The ideal candidate will provide quality patient care, support the clinical team, and ensure adherence to facility standards and protocols. Requirements

• Active LPN/LVN License (required)
• 2 years of recent experience in a nursing home or long-term care setting (required)
• Hands-on experience with PointClickCare EMR (required)
• Willingness to assist with CNA duties when necessary
• Strong reliability, punctuality, and attendance record
• No Requested Time Off (RTO) during the assignment
• Candidates must demonstrate consistent attendance (excessive tardiness or absences may result in early termination)

Responsibilities

• Provide direct patient care and administer medications as prescribed
• Monitor patient conditions and promptly report any changes
• Assist in developing and implementing patient care plans
• Support CNA/PCA staff and assist with patient care duties as needed
• Maintain accurate and timely documentation using PointClickCare EMR
• Ensure patient safety, comfort, and well-being throughout the shift
• Follow all facility policies, procedures, and infection control guidelines
