Become a part of our caring community and help us put health first Conviva is a Subsidiary of Humana, seeking a dedicated LPN/LVN, in this role you will be a part of the Clinical Care Team engaging with our patients to develop lifelong well-being and health. The LPN/LVN role makes a difference in the patient care we offer and assists physicians and other clinicians by providing a unique blend of skills and knowledge to carry out prescribed medical treatment. LPN/LVN are multiskilled health professionals responsible for performing administrative and clinical tasks in our primary care facilities while delivering outstanding customer service and maintaining positive patient engagement.

Requirements

• Current state licensure as an LPN or LVN in the state of Florida
• Bilingual in English and Spanish, with the ability to speak to patients in both languages
• 1+ years' experience as LPN or LVN in an inpatient or outpatient clinic setting
• IV Certified
• Experience in a fast pace/high volume environment
• Experience with Electronic Medical Records
• Must be able to demonstrate strong clinical skills

Nice To Haves

• Hands-on professional Phlebotomy experience
• Experience with HEDIS
• Work closely with providers
• Experienced in wound care
• Demonstrate critical thinking skills
• Knowledge of primary care medicine requirements (state specific).
