About The Position

This position is for a Licensed Practical Nurse / Licensed Vocational Nurse working 12-hour day shifts from 6:00 AM to 6:00 PM in a Long-Term Care or Skilled Nursing Facility setting. The LPN/LVN provides direct nursing care to residents requiring ongoing medical support due to chronic illness, rehabilitation needs, or age-related conditions. The role focuses on promoting patient safety, comfort, and quality of life while assisting residents with medical and personal care needs. The nurse will work closely with registered nurses, certified nursing assistants, and other healthcare professionals to deliver coordinated patient care. Responsibilities include monitoring resident health conditions, administering medications, and supporting care plans developed by the clinical team. The position requires strong patient interaction, attention to detail, and the ability to manage multiple residents within a long-term care environment.

Requirements

• Candidates must hold an active Licensed Practical Nurse or Licensed Vocational Nurse license in the appropriate state and maintain compliance with all applicable regulatory requirements.
• Prior experience in a long-term care facility, skilled nursing facility, or rehabilitation setting is preferred.
• Applicants should demonstrate knowledge of geriatric nursing care, medication administration, and infection control practices.
• Strong communication and interpersonal skills are essential for working with residents, families, and interdisciplinary healthcare teams.
• Candidates must also have the ability to document patient care accurately within electronic medical record systems and follow facility policies related to patient safety and regulatory compliance.
