Requirements
- Current LPN license in the state of Louisiana
- Minimum of 1-2 years of hospice experience
- Strong clinical judgment and the ability to work independently in a home setting
- Compassionate approach to end-of-life care and support for families
- CPR certification, valid driver's license, reliable transportation, and auto insurance
- Knowledge of state and federal home health regulations. Clinical proficiency in nursing, strong communication skills and compassion
Responsibilities
- Respond promptly to after-hours hospice patient needs, including urgent symptom management
- Deliver compassionate nursing care in patients' homes or facilities, honoring comfort, dignity, and peace
- Support families and caregivers with education, reassurance, and emotional presence
- Collaborate with interdisciplinary hospice team members to ensure continuity and quality of care
- Maintain thorough documentation in accordance with state, federal, and company standards
