About The Position

Requirements

  • Current LPN license in the state of Louisiana
  • Minimum of 1-2 years of hospice experience
  • Strong clinical judgment and the ability to work independently in a home setting
  • Compassionate approach to end-of-life care and support for families
  • CPR certification, valid driver's license, reliable transportation, and auto insurance
  • Knowledge of state and federal home health regulations. Clinical proficiency in nursing, strong communication skills and compassion

Responsibilities

  • Respond promptly to after-hours hospice patient needs, including urgent symptom management
  • Deliver compassionate nursing care in patients' homes or facilities, honoring comfort, dignity, and peace
  • Support families and caregivers with education, reassurance, and emotional presence
  • Collaborate with interdisciplinary hospice team members to ensure continuity and quality of care
  • Maintain thorough documentation in accordance with state, federal, and company standards
