About The Position

Canyon Vista Medical Center, part of Lifepoint Health, is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a part-time night shift position in their Acute Rehab unit. The medical center is located in Sierra Vista, Arizona, and is a 106-bed Joint Commission Accredited facility offering various specialty services, including a Level III Trauma unit, Center of Excellence for Total Joint Replacement, Level I Nursery, and an American College of Radiology Accredited Facility. Requirements

  • Current Arizona or compact state LPN or LVN license
  • Associate’s degree from an accredited nursing school
  • Graduate of a Practical Nursing Program required
  • Must have an active AZ or Compact LPN or LVN License
  • AHA BLS required (If you have other than AHA, you will have 30 days from your date of hire to obtain your AHA BLS)

Nice To Haves

  • 1 to 2 years of experience preferred

Responsibilities

  • Assures quality of care by measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital philosophies and standards of care set by Arizona State Board of Nursing, and other governing agency regulations.
  • Coordinates the delivery of patient care for the Rehab Unit, facilitates problem solving with staff members and communicates necessary issues to nursing administration and the Program Manager.
  • Contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care.
  • Administers medication per scope of practice.
  • Documents patient care given.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • Adherence to all core measure initiatives, patient safety goals, and NQF Safe Practices.
