About The Position

Requirements
- Must hold active LPN/LVN license in practicing state.
- LTC/SNF/Assisted Living experience preferred.
- Strong clinical, communication, and care skills.
- Able to follow care plans and collaborate with staff.
Nice To Haves
- LTC/SNF/Assisted Living experience preferred.
Responsibilities
- Assist with ADLs and monitor resident health status.
- Administer medications and treatments.
- Document resident care and report all changes.
- Follow physician orders and established care plans.
- Promote a safe, clean, and supportive environment.
- Provide emotional support and companionship.
