About The Position

We are seeking a Licensed Practical Nurse to join our team as the LPN Long-Term Care Nurse Manager. The LPN Long-Term Care Nurse Manager will oversee daily clinical operations, ensure the highest standards of resident-centered care, and provide leadership to our nursing staff. The ideal candidate will have experience in Point Click Care and NAC (Nurse Assessment Coordinator). The LPN Long-Term Care Nurse Manager will be responsible for managing resident assessments, restorative nursing programs, supervision of staff, and coordination with hospice and ancillary services.

Requirements

  • Valid Licensed Practical Nurse (LPN) license in the state of Pennsylvania.
  • Minimum 3–5 years of nursing experience in long-term care or geriatrics.
  • Strong critical care and long-term care experience.
  • Excellent communication and interpersonal skills.
  • Knowledge of healthcare regulations and compliance standards.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Commitment to providing compassionate patient-centered care.
  • Demonstrated experience with Point Click Care.
  • Experience as a LNAC.
  • Prior leadership or management experience preferred.
  • Strong communication, leadership, and organizational skills.
  • Knowledge of regulatory requirements and best practices in long-term care.

Responsibilities

  • Oversee and manage resident care and assessments for long-term care.
  • Direct and supervise nursing staff to ensure high-quality, compassionate resident care.
  • Lead and support restorative nursing programs to enhance residents’ quality of life.
  • Coordinate care with hospice providers and ancillary services to meet residents’ holistic needs.
  • Collaborate with residents, families, and interdisciplinary teams in developing and implementing care plans.
  • Facilitate communication between patients, families, and healthcare teams.
  • Promote a culture of safety and continuous improvement.
  • Maintain compliance with state, federal, and organizational standards.
  • Serve as a role model for mission-driven, resident-focused care.
