The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles for the resident in the relevant clinical setting and practices within the full scope of nursing expertise/knowledge. They utilize appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. The LPN possesses the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. They ensure the health, comfort and safety of residents, contribute to the assessment of residents, and administer medications or treatments as ordered. The LPN also provides technical support to healthcare professionals as needed and documents resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary. They are skilled in establishing and maintaining effective working relationships with residents, healthcare providers, and the public, and are able to communicate clearly while maintaining confidentiality of sensitive information. The LPN counsels residents, family members, and/or resident advocates on prevention and treatment plans under the direction of a registered nurse, advanced practice provider, or physician. They also have knowledge of resident education principles to develop a self-care program and perform other related duties as assigned.
