LivinRite is seeking an innovative and purpose driven individual to join our team as a full-time Licensed Practical Nurse, who will work collaboratively under the direction and supervision of Registered Nurse. The LPN is responsible for providing total health care services for patients after admission to discharge. He/she provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of this position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness. The LPN acts as a liaison between patients, family members and other health care personnel to ensure efficient and appropriate home health care services. Why join our team? Requirements

Graduate of an accepted Licensed Practical Nursing Program and licensed to practice nursing issued by the state of Virginia
Minimum of two (2) years experience in home health, acute care or rehabilitation setting, preferred.
Current BLS CPR certification.
Must be able to direct care appropriately to support the specific populations (adult & geriatric) provided in nursing services.
Experience in office procedures:
Ability to work independently with minimal supervision.
Ability to resolve problems and set priorities.
Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail.
Pleasant telephone demeanor with exceptional customer service skills.
Basic typing and computer skills.
Ability to work cohesively with others and multi-task in a busy office setting.
Current and active drivers license and car insurance.
