You will oversee daily clinical operations, collaborate with an interdisciplinary team, and ensure that care is delivered according to regulatory standards and resident needs. Requirements

  • Active LPN license in the state of Vermont (or eligibility for reciprocity)
  • Strong communication skills, both verbal and written
  • Exceptional customer service and problem-solving abilities
  • Ability to supervise and work effectively with other personnel
  • Caring and respectful attitude toward residents
  • Physical stamina to meet the demands of the position
  • Proficient computer skills
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

Responsibilities

  • Administer medications and treatments as ordered by the physician, utilizing the five rights of medication administration
  • Plan, implement, and evaluate resident care
  • Direct and monitor the personal care duties and nursing care procedures carried out by Nursing Assistants
  • Assist Nursing Assistants with direct care of residents, as needed
  • Report changes in resident condition, unusual symptoms, daily needs, and progress to the Unit Manager, Assistant Unit Manager, or Charge Nurse Supervisor
  • Follow each resident’s Plan of Care and ensure Nursing Assistants are performing care according to the plan
  • Receive, store, and properly manage controlled substances and other medications; ensure safe placement and use of medications in carts, cupboards, refrigerators, and narcotic cabinets
  • Document in Nursing Notes, Medication Administration Records, Treatment Records, care plans, and all other required records
  • Promote a safe, respectful, and resident-centered care environment
