About The Position

Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. PACE Program The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. LPN Clinic Nurse Provides direct nursing care to participants within the clinic setting as part of an interdisciplinary care team. The LPN monitors participants’ health status by checking vital signs, observing for changes in condition, and reporting findings to the supervising Registered Nurse (RN) or provider to ensure appropriate follow-up. They assist in carrying out individualized care plans, perform basic clinical tasks within their scope of practice, administer medications and treatments as ordered, and document all care accurately in the medical record. The clinic LPN also helps coordinate appointments, supports clinical procedures, and communicates regularly with team members to ensure participants receive comprehensive, well-organized care. Through hands-on nursing support and close collaboration with the care team, the LPN contributes to maintaining participants’ stability, safety, and overall well-being within the clinic environment.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the state of Kansas, active and in good standing
  • Graduation from an accredited practical nursing program
  • Ability to provide nursing care within LPN scope of practice under appropriate supervision
  • Strong observation, communication, and documentation skills
  • Ability to collaborate effectively with an interdisciplinary care team
  • Basic computer skills and experience with electronic health records

Nice To Haves

  • Experience working with older adults or individuals with chronic health conditions
  • Previous experience in a PACE program, long-term care, home health, or clinic setting
  • Familiarity with care coordination and person-centered care models
  • Strong interpersonal and relationship-building skills
