About The Position

Role and Responsibilities Administer practical nursing care to patients in accordance with physician s orders under the direction and supervision of a Registered Nurse. Essential Functions Under the direct supervision of a n RN Case Manager , and within the scope of practice for a L icensed P ractical N urse: Provide health education according to the teaching plan established by the RN. Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN. Observe signs and symptoms , and report patient s reactions to treatments including drugs and changes in the patient s physical or emotional condition , to the physician and RN Case Manager . Document any services rendered to the patient, changes noted in the patient s condition , and/or family and home situations. Document communication with RN C ase M anager and physician. Other duties as assigned. Requirements

• Associate's degree required.
• Diploma as a Licensed Practical Nurse from an accredited program.
• Current, valid LPN license issued from NYS Department of Education.

Nice To Haves

• Experience in home health care preferred.
• Community health experience preferred .
