The Licensed Practical Nurse (LPN) position involves basic nursing duties providing care to clients with a variety of health concerns. All direct patient care is done under the supervision of the physician/mid-level providers. The LPN conducts patient education and advocacy where appropriate within the structure of care.

  • Ability to identify and categorize each patient’s ag-specific grouping of needs, such as those for infant, adolescent, or geriatric patients
  • Understand and effectively use the ACHS Electronic Medical System
  • Knowledge of medical procedures and working with medical team to deliver comprehensive care
  • 1 – 2 years of direct experience
  • Graduation from an accredited LPN Program
  • License on file with the state of New Hampshire
  • Current BLS Certification

Responsibilities

  • Works as an integral member of the health care team
  • Greets and prepares patients to see physician/mid-level provider. Collects and records data for health history including height, weight, BP, pulse, temperature, respirations, medication, and allergies as indicated
  • Administers specified medication, orally or by subcutaneous or intramuscular injection, and notes time and amount on patients
  • Performs phlebotomy and is responsible for ensuring appropriate lab slips are prepared
  • Maintains log of lab tests ordered and follows up with reference lab as necessary
  • Assists provider with simple procedures. Acts as chaperone when necessary
  • Assists providers with patient follow up on normal labs
  • Keep storage areas, labs, and exam rooms routinely stocked, safe, clean, and orderly
  • Cleans and sterilizes instruments and any special procedure packs daily
  • Actively attends and participates in health center staff meetings
  • Assists with special clinics such as Well Child, Blood Pressure screening, and Immunization clinics
  • Willing to pursue continuing education opportunities to enhance personal and professional growth
  • Complete and sign all documentation in an accurate and timely manner
  • Must complete work within scope of practice for current licensure or certification
  • Other duties, hours, and ACHS site placement may be assigned
