About The Position

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Requirements

  • Must have completed a requisite practical nursing program or be licensed in the state where the agency is located.
  • Currently licensed as a Licensed Practical Nurse in the state.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.

Nice To Haves

  • Have three years nursing experience preferred.
  • Home Health and/or hospice experience preferred.

Responsibilities

  • Provides direct patient care as defined in State’s Nurse Practice Act.
  • Implements plan of care initiated by the Registered Nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s).
  • Participates in coordination of services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the Registered Nurse, Clinical Services Manager and/or Director of Clinical Services.
  • Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; along with collaboration with the Registered Nurse Case Manager obtains/receives physician’s orders as required.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the Registered Nurse.
  • Prepares clinical notes and ensures documentation is completed on a timely basis.
  • Participates in holiday rotation and on-call duties as required by the agency.
