Requirements

Active LPN license in the state of Indiana
Assisted living, long-term care, or senior care experience preferred
Strong medication management and documentation skills
Compassionate, resident-focused approach to care
Ability to work collaboratively in a team-based environment
Ability to stand, walk, bend, and assist residents as needed

Nice To Haves

Experience in Memory Care or dementia support
Familiarity with electronic health records (EHR)
CPR/First Aid certification (or willingness to obtain)

Responsibilities

Administer medications and treatments in accordance with physician orders and state regulations
Complete nursing assessments and contribute to resident care plans
Monitor resident conditions and report changes promptly
Provide hands-on nursing care in Assisted Living and Memory Care settings
Accurately document nursing services, medication administration, and observations
Collaborate with caregivers, leadership, and healthcare partners
Support resident dignity, comfort, and emotional wellbeing
Follow infection control, safety, and compliance standards
Promote teamwork, communication, and a positive care environment
