The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles for the resident in the relevant clinical setting and practices within the full scope of nursing expertise/knowledge. The LPN ensures the health, comfort and safety of residents and contributes to the assessment of residents, administers medications or treatments as ordered, and documents resident interactions as needed.

Requirements

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Associate degree in practical nursing is preferred.
  • Must have attended a pre-licensure nursing program that included supervised clinical experience.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board.
  • Obtains and maintains required department specific competencies and certifications.
  • Completes required annual continuing education units (CEU) and competency training.

Nice To Haves

  • Experience in long term care settings.
  • IV therapy/management skills if required by position.

Responsibilities

  • Provide professional nursing care for residents of all ages in long term care.
  • Demonstrate knowledge of nursing principles and competency in nursing expertise.
  • Perform testing to evaluate data reflective of the resident's status.
  • Recognize problems, recommend solutions, and respond effectively in emergent situations.
  • Ensure the health, comfort, and safety of residents.
  • Contribute to the assessment of residents and administer medications or treatments as ordered.
  • Document resident interactions and outcomes related to care.
  • Establish and maintain effective working relationships with residents and healthcare providers.
  • Counsel residents and family members on prevention and treatment plans.
  • Develop self-care programs based on resident education principles.
  • Perform other related duties as assigned.
