This position offers Licensed Practical Nurses (LPNs) an opportunity for a change from facility chaos and overloaded assignments, providing one-on-one skilled nursing care in a pediatric home setting. Requirements
- Active IL RN or LPN license (or Compact license)
- Strong assessment and communication skills
- Compassionate, dependable, and self-motivated
- Valid driver’s license and reliable transportation
Responsibilities
- Provide skilled, one-on-one nursing care in the home
- Administer and manage medications
- Monitor and document patient conditions
- Perform treatments as ordered by the physician
- Educate and support family members
- Collaborate closely with the Phoenix clinical team
