Requirements

Current LPN licensure in the state of practice.
Valid CPR and BLS certification.
Previous experience in home health care or a similar setting is preferred.
Strong assessment and critical thinking skills.
Ability to work independently and manage time effectively.
Excellent communication and interpersonal skills.
A genuine compassion for patients and a commitment to quality care.

Responsibilities

Provide direct patient care under the supervision of a registered nurse.
Administer medications and treatments as prescribed by physicians.
Monitor and record patients' vital signs and health status.
Assist with daily living activities, including bathing, dressing, and grooming.
Educate patients and their families about health management and home care.
Maintain accurate and timely patient records and documentation.
Communicate effectively with healthcare team members to ensure optimal patient care.
