About The Position

Glencoe Regional Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our GlenFields Long Term Care team. Our skilled nursing facility, Glenfields, is guided by the household model of care, which means residents live in "households," or living units made up of 18 private rooms, and shared community spaces such as a kitchen, dining room, living room, and sunroom. Unlike in a traditional nursing home, the household model of care emphasizes a person-centered approach where residents have control over their daily lives and living environment. As an LPN on our team, you will play a vital role in ensuring the health, safety, and well-being of our residents. Responsibilities include monitoring resident care, addressing family and visitor concerns, assessing staffing needs, and supporting a positive and safe environment for both residents and staff. You will oversee care delivery in your assigned area(s) and ensure compliance with all facility policies and procedures. Additionally, you will be expected to promptly report any unusual issues or incidents to the appropriate leadership or administrative personnel.

Requirements

  • Must be graduate of an accredited school of nursing and possess a current License Practical Nurse license from the State of Minnesota. License must be in good standing.
  • Must have current BLS healthcare provider certification.
  • A true passion for working with Gerontological Nursing.
  • Provides patient care with empathy, understanding, patience, tact, and professionalism.
  • Strong communication skills are required. Must be able to communicate with tact and professionalism.
  • Is a supportive team member. Contributes to and is an example of teamwork and team concept.
  • Ability to multitask, prioritize and organize workload according to resident and department need.
  • Proficient in Windows-based computer software. EMR experience a plus.
  • Actively supports GRH mission, quality improvement goals and adheres to core values and behavioral standards.
