About The Position
We are seeking a compassionate and experienced Level 3 Licensed Practical Nurse (LPN) with DDI experience to provide high-quality care in a residential or community-based setting. The ideal candidate will have hands-on experience with G-tube feedings and tracheostomy care and a strong commitment to supporting individuals with developmental disabilities.
Requirements
- Active LPN license in MD
- Proven experience with G-tube and trach care
- Previous experience working with individuals with developmental disabilities (DDI)
- Strong documentation and communication skills
- Reliable, professional, and compassionate
Responsibilities
- Provide skilled nursing care, including G-tube feedings and tracheostomy care
- Monitor and document client health status and report changes as needed
- Administer medications and treatments per physician orders
- Collaborate with the healthcare and direct support teams to ensure safe, person-centered care
- Ensure compliance with DDI and state regulatory requirements
