About The Position

The LPN IV is responsible for providing nursing care, administering medications, and documenting care in the patient's medical record for a designated group of patients under the direction of a medical provider and administrative supervision of the Practice Manager. This role involves utilizing the nursing process and age-specific needs on an assigned shift. The employee is subject to call back and overtime as required by the hospital. Key duties include performing specified patient care with skill, organizing patient flow, assisting patients with various needs (walking, transfers, dressing), collecting specimens, preparing exam rooms, collecting patient history, performing screenings and lab tests, assisting medical providers with procedures, charting, relaying instructions to patients and families, and answering telephone calls. The LPN IV will administer medications and treatments within their scope of practice, prepare for surgery, and assist with procedures and examinations according to policy. A critical component is initiating peripheral IV access and administering IV flushes and IVPBs. The role also mandates ensuring the proper segregation and safe disposal of hazardous pharmaceutical waste, receiving and transcribing physician orders, contributing to patient care plans, identifying and reporting changes in patient condition or adverse reactions, and documenting all care. Environmental responsibilities include cleaning and sterilizing exam/procedure room instruments and equipment to maintain infection control, keeping records, ordering, sorting, storing supplies, and restocking rooms. Organizational duties involve respecting and promoting patient rights, adhering to clinic and system-wide policies, responding to medical emergencies (including initiating CPR), ordering patient supplies, completing referrals, fostering a positive work environment, and performing preceptor duties for new employees during orientation.

Requirements

  • Completion of a vocational school LPN program.
  • Completion of a formal (school or agency) intravenous therapy-training program.
  • Current licensure as a Licensed Practical Nurse as approved by the Tennessee Board of Nursing for practice in the state of Tennessee.
  • Responsible for Continuing Education Units annually.
  • BLS certification must be obtained within the orientation period, and certification must be kept current while in this position.
  • Proficiency in pharmacology and documentation and the ability to demonstrate nursing skills according to current hospital and nursing policies and procedures as normally acquired through the successful completion of documentation class, pharmacology exam, and skills exam.
  • Must have a minimum of 6 months experience as a Licensed Practical Nurse.
  • Must have attended a formal IV push medication training program prior to administering IV push medications.

Responsibilities

  • Performs specified patient care with a high degree of skill, minimal discomfort to the patient, and minimal supervision.
  • Checks schedules; organizes patient flow accompanying patients to exam/ procedure room; assists patients as needed with walking, transfers, dressing, collecting specimens, preparing exam room/ setting up instruments etc.; collects patient history; performs screenings/ lab test per Medical Provider’s orders; assists Medical Provider/ other nursing staff with various procedures; charting; relays instructions to patients/ family; answers telephone calls and provides pertinent information.
  • Administers medications and treatments within scope of practice, prepares for surgery and assists providers with procedures and examinations according to policy.
  • Prepares for tests and surgery, and assists physicians with procedures and examinations according to policy.
  • Initiate peripheral IV access and administer IV flushes and IVPBs to patients according to policy.
  • Ensures the proper segregation and safe disposal of hazardous pharmaceutical waste.
  • Receives and transcribes the physician, FNP, PA orders as appropriate.
  • Contributes to the development, implementation, and continuing evaluation of the patient care plan.
  • Identifies, records, and reports any change in patient condition or adverse reaction to treatment and/or medication to the medical provider.
  • Documents care in patient’s record.
  • Fulfills environmental duties to include cleaning exam/procedure room instruments and equipment between patient visits to maintain infection control; sterilize any equipment or instruments according to scheduled maintenance program; keeping appropriate records; ordering, sorting, storing supplies; restocking exam/ procedure rooms.
  • Fulfills organizational duties to include respecting and promoting patient rights; following all Clinic Policies and Procedures and System wide policies.
  • Responds to medical emergencies, initiates CPR or appropriate action, and notifies appropriate personnel.
  • Orders patient supplies and completes referrals for patients.
  • Performs related responsibilities as required or directed.
  • Contributes to a positive work environment and the overall team effort of the clinic.
  • Performs preceptor duties to new employees to provide needed guidance during orientation.
