LPN - Integrated Health Center
About The Position
Under the direct supervision of the Director of Nursing, this position will provide customer service while coordinating services provided in the integrated health center. Employees are expected to perform essential duties to performance standards. Requirements
- Possession of a High School diploma or the equivalent.
- Successful completion of, an accredited Licensed Practical Nursing program (Diploma or Associate’s degree) that includes clinical training (vital signs, injections, point-of-care lab testing, EKGs, provider assistance, etc.)
- Must possess and maintain a reliable personal vehicle.
- Strong customer service skills.
- Ability to work flexible hours.
- Applicant must successfully pass required criminal background check and drug test.
- Ability to document personal identity and employment eligibility within three (3) days of original appointment as a condition of employment in compliance with Immigration Reform and Control Act [I-9] requirements.
Responsibilities
- Serve as the primary care clinic contact for the facilitation of client service-related issues
- Room patients, complete vitals and gather all needed medical information for provider’s examination
- Perform ordered procedures such as spirometry, EKG’s, blood glucose testing, hemoglobin A1c, lipid panel and urinalysis.
- Call prescription information into pharmacies and problem solve pharmacy-related issues
- Contact emergency services as directed
- Perform basic clerical duties, e.g., copying, faxing, shredding
- Assist in medication handling processes
- Provide clerical assistance to medication technician as needed
- Document all patient care in electronic record and maintain current clinical records in accordance with agency policies and procedures
- Re-stock rooms with needed medical supplies.
- Assists with prior-authorizations for clients’ specialty referrals
- Provide referral and scheduling information regarding primary care services available on site and in the community
- Provide clerical assistance to support primary care as needed
- Assume other duties as assigned to include assistance in other areas of agency
