About The Position

Schedule: On call requirements of one 12 hour weekend shift/month and two evenings. Weekends are 8a-8p and 8p-8a. Evenings are 4:30p - 10:30p. Job Summary: Provides care to patients under the direction of a registered nurse or physician, functioning within the scope of license.

Requirements

  • Graduate of an accredited school of nursing or educational equivalency for licensing
  • One year licensed practical nurse experience
  • State of Work Location: Illinois Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA) And Driver’s License: State of Issuance And Driving Record - Motor Vehicle Report (MVR) And Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR) State of Work Location: Missouri, Oklahoma, Wisconsin Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA) And Driver’s License: State of Issuance And Driving Record - Motor Vehicle Report (MVR) And Licensed Practical Nurse (LPN) Nurse Licensure Issued by Compact State Or Licensed Practical Nurse (LPN) - Missouri Division of Professional Registration

Responsibilities

  • Delivers care to assigned clients and performs direct care in accordance with an established plan of care.
  • Delivers safe care at all times that is in line with physician orders, home health policy and established nursing care standards.
  • Maintains communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation.
  • Reports patient-related observations to supervising RN on a daily basis.
  • Administers medications and observes patients for adverse reactions to medications or treatments.
  • Provides documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirements and ensures reimbursement.
  • Works in a constant state of alertness and safe manner.
  • May perform point of care testing according to policies and procedures.
  • Performs other duties as assigned.
